A fantastic opportunity to join our Medical Assessment Unit, this is a busy 22 bedded assessment area, with an additional triage unit, taking admissions from GP and the Emergency Department, which will provide the opportunity to develop your knowledge and skills within acute medicine.
You will work with other members of the MDT to both organise and facilitate care appropriate to the individual needs of the patient.
Main duties of the job
1. Deliver high quality, compassionate care under the direction of a Registered Nurse (or other registered care professional dependent on setting) with a focus on promoting health and independence
2. Have proficient attitudes and behaviours compatible with NHS Values
3. Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual
4. Carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of a registered nurse or other registered care professionals dependent on setting

Detailed job description and main responsibilities
-Contribute to the assessment, implementation and evaluation of care to meet patients’ needs in accordance with agreed local and national standards and evidence based guidelines under supervision.
-Support and provide patient education and health promotion.
-Support the delivery and co-ordination of care, through appropriate professional practice.
-Contribute to the management of risk and maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines.
-Assist in setting, monitoring and maintaining standards of care to improve the patient experience and outcome of care.
-Communicate all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care.
-Maintain accurate and complete records of all care activities, in a timely manner, adhering to local and professional standards for documentation.
-Act as a role model, presenting a positive image of self, team and the organisation.
-Promote equality in care and practice by recognising, respecting and meeting the needs and choices of individuals.
-Ensures environment of care is safe, well equipped and compliant with standards for hygiene and cleanliness.
-Maintain and develop own professional competence and skills within scope of role.
-Actively participates in the appraisal and Personal development planning process as reviewee.
-Proactively supports Trust wide or local changes and developments in nursing practice e.g intentional rounding, alcohol screening as directed etc.
-Actively try to resolve issues and complaints at local level and then report the outcome to line manager.

Qualifications & Education
Essential criteria
5. •Completed Foundation Degree in Health & Social Care OR •Achieved LTHTR Assistant Practitioner Programme.
6. •Maths & English grade 4-9 or grade C or above •OR •Functional Skills Maths & English Level 2.
Desirable criteria
7. •Basic Life Support Cascade Trainer
8. •Key Mover Cascade Trainer
9. •ANTT Assessor
10. •Good IT skills
Knowledge & Experience
Essential criteria
11. •Experience of working as a Trainee Assistant Practitioner within an acute hospital setting.
12. •Proven competency of performing a core range of clinical procedures.
Desirable criteria
13. •Experience of working as an assistant practitioner within an acute hospital setting
Skills and Abilities
Essential criteria
14. Ability to deliver a high standard of nursing skills and quality patient care
15. The ability to communicate effectively with patients, visitors and the MDT.
16. Can work effectively as part of a team.
17. Can delegate appropriately.
18. Provides effective supervision of junior members of staff.
