Overview

The Leeds Centre for Older Peoples Medicine is seeking to appoint a Locum Consultant Geriatrician. The successful candidate will work as part of the Older Peoples Medicine team delivering inpatient care for patients on the orthogeriatric wards at the LGI, delivering Bone Health clinics and outlier wards within the Trust. There may be the opportunity to provide some sessions on the Same Day Emergency Care unit. This can be discussed at appointment. The post will be 10 PAs but we are willing to discuss less than full time. The initial appointment will be for 6 months with a view to extension.


Main duties of the job

The post focuses on the delivery of care of Older People. To ensure high quality and safe care of elderly medical patients resulting in excellent clinical outcomes. To contribute to the care of patients on the older peoples orthogeriatric and specialty wards thereby extending Geriatrician presence throughout the usual working week. To contribute to out-of-hours provision on a rota. To show clinical leadership in patient care, service development and clinical governance. To work cooperatively as part of the wider elderly medicine team, being held to account for clinical, service development and training outcomes. Applicants must be able to demonstrate a high level of clinical experience and competence in all aspects of Older Peoples medicine. Excellent communication and team working skills, a strong commitment to teaching and service development are essential. The successful applicants should hold accreditation in Membership of the Royal College of Physicians or equivalent, and should be on the Specialist Register.

The post will be based at St James's University Hospital, and will be predominantly clinical with responsibility as a member of consultant team for providing inpatient care for people living with medical and frailty conditions. Consultant mentoring will be offered.

Job description / Responsibilities

The post will be based at both SJUH and LGI. The Elderly Medicine admission wards are based in Gledhow Wing, St James Hospital, with between 26-29 beds per ward. At least two consultants conduct post-take ward rounds on each admission ward during the week. Each ward is supported by a registrar, two core medical trainees or FY2 trainees and one FY1 trainee. On-call duties: Monday to Thursday the Elderly Medicine consultants are on call on a 1:20 rota. In winter, three Elderly Medicine consultants participate in ward rounds on the acute admissions wards; only one is on call overnight. In summer, two conduct weekend ward rounds. Since 2018, an evening ward round 17:00-20:00 is expected for the first on-call consultant. The postholder would work six weekends in a 12-month period. The post holder will work with Orthogeriatric and orthopaedic teams to provide high-quality care for older people presenting with fractures, with focus on fractured neck of femur, perioperative medicine and bone health. They will liaise with consultant colleagues in other specialist teams within the Trust, emergency department, other medical and acute surgical specialties, and primary care to provide excellent care for older people living with frailty. The postholder would spend time in the SDEC Unit in the Emergency Department alongside the Leeds Integrated Discharge Service (LIDS) to help identify and assess patients suitable for discharge directly from the ED. Each elderly ward in Gledhow Wing has 27-30 beds; the postholder would share the ward with other consultants, covering each others’ patients when colleagues are on leave. Patients in beds outside the CSU are allocated to consultants depending on ward location. The Community Care Bed (CCB) unit includes rehabilitation units around Leeds; a GP VTS trainee supports the post holder. Neighbourhood work: Leeds is divided into 13 neighbourhoods that bring health and social care services together to improve community services. The Elderly Medicine consultant supports these teams by attending meetings and performing domiciliary visits if required.


Person Specification

Eligibility, Fitness to practice, and Language Skills

* Eligible for full registration with the GMC at time of appointment and hold a current licence to practice.
* Eligibility to work in the UK.
* All applicants must have demonstrable skills in written and spoken English adequate to enable effective communication about medical topics with patients and colleagues.

Clinical, Academic, and Personal Skills

* Well organised and skilled in good time management. Effective interpersonal skills.
* Demonstrate ability to relate to and work within a team.

Desirable

* Service Development Experience. Attendance on a recognised management course.

Qualifications

* MB BCh/MBBS (or equivalent) MRCP or equivalent qualification.

Desirable

* Higher degree e.g. MD/PhD relevant to the subspecialty.

Clinical Experience and Career Progression

* Must be able to demonstrate a high level of clinical experience and competence in older peoples medicine.
* Must be able to demonstrate a high level of clinical experience and competence in older peoples medicine with evidence of continuous career progression consistent with personal circumstances.
* Experience of participation in regular clinical audit.
