Job Summary
Band 7 - Clinical Therapy Lead – £49,387 – £56,515. Integrated Transfer of Care (ITOC) team. Hours per week: 37.5. Service runs 0800‑1800 across seven days, with occasional weekend duties for the Clinical Lead. Flexible working hours and permanent contract.
Key Responsibilities
* Provide therapy assessments and equipment to patients in A&E, Frailty and the Recovery Hub.
* Support the ITOC service, including triage and review of complex patients.
* Work in partnership with the Clinical Nurse Lead and Team Leader to deliver the therapy aspect of the ITOC team.
* Contribute to multidisciplinary re‑ablement and rehabilitation plans, and support short‑term care and support when required.
* Aim to reduce time in hospital, prevent hospital‑acquired infections and de‑conditioning, and promote timely, safe discharges to appropriate care settings.
