Job overview
An exciting opportunity has arisen for an enthusiastic and experienced Nurse or Allied Health Professional to join the Kerrier Homefirst team, based in Camborne Health Office.
This position is open to Nurses, Occupational Therapists and Physiotherapists with experience of working in the community who wish to develop their assessment, care planning and reablement skills whilst being part of a progressive, integrated multiagency team.
The Homefirst team is part of the Integrated Community Team in Kerrier, comprising registered and non-registered nursing, therapy and administrative staff. Working in partnership with social care, GPs and voluntary agencies, we support intermediate care and rehabilitation needs, helping people to regain independence and remain at home wherever possible.
You will be responsible for assessment, planning and implementation of care, interpreting information relevant to the patient's condition and progress, identifying needs and setting realistic, person-centred goals. You will need excellent organisational skills, a broad clinical knowledge base and experience of older persons' services.
This post provides an excellent opportunity to broaden your skills and apply a holistic approach to patient care in the community. It involves regular weekend working, and the successful candidate must be flexible with working days.
Main duties of the job

Main duties:
* To provide a high standard Occupational, Physiotherapy and Nursing to the service, performing advanced assessments of patients including those with highly complex presentations, and from this formulating a diagnosis and treatment plan.
* To operate as an advanced practitioner being first contact for urgent requests to help both prevent hospital admission and facilitate early discharge.
* To utilise advanced skills in community assessment and management of the acutely sick patient.
* To provide leadership for support staff and junior staff through supervision, training and appraisal.
* To participate in the planning and development of occupational services within the designated team.
* Model and deliver excellent evidence based clinical care, working as an autonomous Practitioner.

Education/Qualifications
Essential criteria
* Degree or diploma in Occupational Therapy or Physiotherapy
* HCPC registered, NMC Registered
* Evidence of appropriate postgraduate courses and CPD to support practice, to degree equivalent level
Knowledge and Abilities
Essential criteria
* Confident and competent leadership with the ability to work in diverse and complex situations
* Proven leadership qualities, including the ability to motivate a team and negotiation skills
* Ability to make clinical risk-management decisions in lone-working situations
* Awareness of and ability to respond to the pressure of political change in health and social care systems
Skills and Aptitude
Essential criteria
* Clear understanding of clinical effectiveness and the need to provide evidence based care
* Highly developed verbal and written communications skills that will enable the post holder to work effectively across boundaries and in a variety of settings
* Imparting bad news to patients and their families/carers; demonstrating the ability to communicate in a form and manner that is consistent with the level of understanding, culture, background and preferred communications method of each individual
* IT Literate

* Teaching, Assessing, coaching and mentoring skills to facilitate an effective learning environment for staff and students

* Substantial post registration clinical experience in a similar role
* A broad range of clinical experience in a variety of community and inpatient settings
