What you will do in this role:

1. Deliver neuro-rehabilitation programmes in outpatient, home, and community settings under the guidance of qualified therapists.
2. Support Physiotherapy and Occupational Therapy interventions.
3. Monitor patient progress and contribute to therapy planning within agreed guidelines.

Main duties of the job

8. Support neuro-rehabilitation by implementing therapy programmes devised by the lead therapist
9. Manage your own patient caseload, assessing and treating patients, and monitoring their progress.
10. Assist in patient assessments, gathering relevant information, and adapting treatment plans under therapist guidance.
11. Work closely with the multidisciplinary team, contributing to patient care planning and discharge processes.

Detailed job description and main responsibilities

15. To carry out basic assessments of patients’ needs as directed by Qualified Clinicians
16. To carry out mobility and transfer assessments for specific pieces of equipment as requested by therapy or nursing staff and report back to qualified member of the team regarding the need for further input.
17. To be able to identify more complex issues that need assessment by qualified clinician.
18. To implement therapy programmes (as prescribed by the OT or PT) both to individuals and provide feedback to therapists.
19. To carry out programmes devised by therapists using a range of techniques which the post holder has been deemed competent to do.
20. To review patient progress and provide feedback at all stages of patients progress in caseload discussions.
21. To work jointly with clinicians in cases which are complex and may require physical moving and handling as part of their rehabilitation.
22. To be responsible for assessing and managing clinical risk in relation own caseload, seeking guidance where appropriate.
23. Will not attempt any procedure without appropriate training and being deemed as competent to carry it out
24. To contribute to the evaluations of therapeutic interventions including planning discharge:
25. To set/ review SMART goals with patients, adapting them as the patient progresses.
26. To be able to monitor progress with appropriate outcome measures, adjusting treatment as appropriate.
27. To be able to prioritise work - load:
28. To plan day-to-day workload.
29. To be able to identify factors involved in making decisions with regard to prioritisation.
30. To be able to prioritise the referrals to rehabilitation assistants on a daily basis.
31. To provide support to clients, their carers, and other professionals involved in a patients care journey:
32. To use listening skills and be aware of the issues and of how to respond to questions and anxieties. This requires empathy, sensitivity and good interpersonal skills, especially when a patient has barriers to understanding.
33. To liaise with family, carers or other involved with a patients care in order to teach exercise programmes for them to supervise.
34. To participate in the multi-disciplinary team within the Dorking Primary care Network Service.
35. 7. To adhere to organisational standards for documentation / writing notes:
36. To maintain timely patient progress notes on EMIS / Systm1
37. To abide by the record keeping standards of SDHC
38. To be involved in service projects and developments
39. To contribute to and support any projects for improving the Community Therapy service.
40. To keep clinical skills updated by attending study days and training.
41. To maintain personal and professional development
42. To take part in CPD
43. Attend in-service training above.
44. To participate in SDHC’s appraisal scheme and formulate a personal development plan.
45. To participate in Clinical supervision.
46. To fulfil any specific tasks as requested by the Team Leader or other therapist
47. To maintain complete confidentiality of all information concerning patients that may be known or acquired in the course of work as per SDHC policy.
48. To adhere to all policies and procedures of the Community Therapy Service.

49. GCSE or equivalent English and Maths
50. Broad based educational achievement

Experience and Knowledge

Essential criteria

52. Experience working in the NHS or similar healthcare settings
53. Experience working with families and carers in a supporting role
54. Knowledge and understanding of safeguarding responsibilities, including the duty to protect children and vulnerable adults and actively promote their welfare.

Desirable criteria

55. Experience working with individuals with cognitive, communication, or behavioural impairments
56. Experience working in a rehab setting
57. Experience in using alternative communication strategies (e.g., visual aids, supported conversation approaches)

59. Able to work effectively with minimal supervision in a busy environment
60. Ability to work within agreed competency framework
61. Computer literate, with the ability to use clinical systems following appropriate training
