Overview
Support medication reviews, conduct medicines reconciliation, identify patients for medication changes, assess prescribing data, and implement initiatives to improve medication safety and compliance within the Brownlow Health Primary Care Network.
Job Responsibilities
* Facilitate medication reviews and resolve prescribing queries to improve patient care.
* Support medication review and medicines reconciliation processes.
* Identify patients suitable for medication changes aligned with the Practice prescribing teamwork plan and implement changes with GP or pharmacist agreement (e.g., dose optimisation, formulary switching).
* Analyse prescribing data using clinical computer systems, highlight recommendations, and implement necessary changes to keep patient records up to date.
* Assist with medication changes from hospital letters and new patient reconciliation under lead pharmacist/GP direction, and report concerns to the PCN pharmacist/GPs for local clinical governance (e.g., Datix reporting).
* Implement waste‑reduction initiatives, improve medication compliance, and ensure best practice according to local guidelines, legislation, and safety updates (NICE, MHRA).
* Conduct High‑Risk Drug Monitoring and provide advice and support on all prescribing issues.
* Contribute to Quality and Outcomes Framework (QoF), Impact and Investment Targets (IIF), Direct Enhanced Services Contract (DES) and CPPE Primary Care Pathway, and support the Brownlow Health Frailty team.
* Promote high‑quality, safe, and cost‑effective prescribing informed by evidence‑based practice and national/local guidance.
* Collaborate with multidisciplinary teams, including GPs, nurses, pharmacists, and other healthcare professionals.
* Work at all times within professional limits and influence prescribing behaviour across the practice MDT.
Qualifications
* General Pharmaceutical Council registration as a Pharmacy Technician.
* BTEC National Diploma in Pharmaceutical Science, NVQ level 3 Pharmacy Services, or equivalent.
* Evidence of CPD.
Experience
* Previous experience working as a Primary Care Pharmacy Technician.
* Experience with car access and a driving licence.
