Overview
Mental Health Patient Flow Co-ordinator responsible for supporting seamless patient journeys from assessment to discharge across East Lothian acute and community settings. Provides expert clinical input, senior advisory support to the Service Manager, and leadership within the Mental Health Team to optimise patient flow and avoid unnecessary admissions.
Responsibilities
* Rapidly analyse assessments for each patient and synthesise a cohesive discharge plan involving Psychiatry, Nursing, Occupational Therapy, and Adult Social Work as appropriate.
* Provide senior clinical support to the Service Manager through day-to-day co-ordination of the Mental Health Team with a focus on patient flow.
* Offer highly specialist clinical advice to healthcare professionals (IHTT, CMHT, CWIC, PTS, Psychiatry), other agencies, carers, clients and relatives.
* Support the Clinical Nurse Manager in enhancing and developing existing services and any new services to meet local community needs.
* Work with Team Leads to deliver services and deputise into leadership/management roles in Team Lead absence.
* Collaborate with clinical teams to develop and deliver care programmes, including assessment, planning, implementation and ongoing evaluation for patients in the domiciliary setting and in the East Lothian community.
* Act as the key clinical lead when working with health and local authority colleagues to prevent hospital admissions and to ensure early and effective discharge.
* Lead and develop multi-agency teams to improve practice, implement and evaluate evidence-based standards, guidelines and policies.

* Collaborate with Primary and Secondary Care Teams, Unscheduled Care Service and other agencies involved in community-based care.
* Lead cross-agency efforts to improve patient flow and discharge planning across settings.
