Personal Advisor – Leaving Care – Telford and Wrekin
Pay rate £21.25 per hour
Contract role, Personal Advisor.
Experience within a similar role is essential.
- Undertake the statutory role of Personal Adviser role for individual Care experienced young people from age 16 up to the age 21 or 25.
- To be the key professional and case manager to Former Relevant care experienced young people and when needed to those young people who are Relevant or Qualifying for advice and assistance
- To work with young people leaving care and their support network to support the development of independent living skills and ensure a smooth transition to independence
- To assist and support young people to prepare for leaving care and living independently; providing support, advice and guidance to them up to age 25.
