Mental Health Support Coordinator
North Tyneside Area, North Shields and community based.
Permanent full time (37.5 hours per week)
Pay £12.61 per hour (£24,664 per annum) and great benefits including Health Cash Plan

Typical day as a Lead Support Worker (known by us as Coordinator)
1. Creating support plans with your customers and working closely with social workers, CPN/community treatment teams, probation, and North Tyneside Recovery Partnership to ensure the best wrap-around support for the customer.
2. Helping customers with various needs, including community integration, medication prompting, financial support, benefits assistance, and more.
3. Supporting customers with mental health, learning disabilities, substance misuse, forensic history, and dual diagnosis.
4. Supporting customers under 117 aftercare, helping those who have been sectioned under sections 3, 37, 45A, 47, or 48 of the Mental Health Act 1983 to live independently.
5. Assisting with daily activities such as shopping, budgeting, maintaining tenancy, and transitioning to independent accommodation.
6. Carrying out risk assessments, support planning, goal setting, and reviews.
7. Engaging in informal chats and catch-ups, with ample time for collaboration.

You bring
1. Passion for supporting customers to live their best lives, with collaborative working and attention to detail.
2. Experience in creating person-centred support plans and guiding colleagues in their implementation.
3. Experience in coordinating and assessing customer referrals.
4. Ability to work independently, stay calm under pressure, and demonstrate resilience.
5. A vehicle insured for business purposes, with mileage reimbursed by us.

Job details
1. Based in North Tyneside, working from Railway Terrace, North Shields, with community visits and office work.
2. Flexible start and finish times to meet customer needs.
3. Rotational schedule covering alternative weekends, 8 am to 8 pm shifts.
4. Proficiency with technology for support planning, online learning, and collaboration.
5. Enhanced DBS check required, paid for by us.
