Overview
This post is an exciting opportunity for a Senior Pharmacy Technician to work as a member of the Hospital at Home Team within North Tees & Hartlepool NHS Foundation Trust. The role will focus on promotion of safe, rational, effective, and economic use of medicines used in Hospital at Home patients. The post holder will provide pharmaceutical advice to members of the Hospital at Home multi-disciplinary team (MDT) with the support of the clinical pharmacist team. The post holder will be expected to be an active member of the Hospital at Home MDT, providing feedback to virtual ward rounds when required. There will be an expectation that the post holder will support in delivery of training to members of the Hospital at Home MDT on the safe use of medicines and medicines optimisation and provide responses to queries from nurses and other healthcare professionals, referring any clinical issues to the pharmacist.
This is a newly developed role and will enable the successful candidate to be instrumental in developing and improving a strongly patient-centred approach.
The advert notes that South Tees Hospitals NHS Foundation Trust and North Tees and Hartlepool NHS Foundation Trust now form University Hospitals Tees; you may be required to work at any site across both Trusts.
Key responsibilities
* Provide medicines reconciliation for Hospital at Home patients to promote safe and effective use of medicines.
* Promote safe and effective use of medicines in community settings for people accessing the Hospital at Home service.
* Act as a resource for other members of the MDT and function as an active member of the Hospital at Home MDT.
* Participate in reviewing the effectiveness of the role and how it might evolve to deliver positive outcomes for this group of patients.
* Support delivery of training to Hospital at Home MDT members on safe use of medicines and medicines optimisation; respond to queries from nurses and other healthcare professionals and refer clinical issues to the pharmacist as appropriate.
