Job summary

Community Inpatient Unit provides Rehabilitation, Re-ablement andrecovery interventions for adults in North East Lincolnshire. The service willpromote faster recovery from illness, prevent unnecessary acute hospitaladmissions and premature admissions to long-term care, support timely dischargefrom hospital and maximise independent living wherever possible.Multi-professional staff will collaborate with individuals to ensure theyachieve their full potential in terms of their level of independence, health,and wellbeing.

Main duties of the job

Job title: Advanced Clinical Practitioner

Department: Community Inpatient Unit

Hours: 37.5 Permanent

Salary Range 8A £48,480 - £58,178 per annum

Weare seeking a highly motivated, passionate and committed Advanced ClinicalPractitioner to join the multidisciplinary team at Community inpatient Unit(CIU) within CPG. The post will be based at the CIU and will oversee the safe and effective Nursing oversight, ensuring the service meets the Care QualityCommission Essential Standards. The post holder will work alongside the Unitmanager and lead on the review, development, implementation of servicestrategies including completion of audits, action plans, Incident reviews andquality assurance.

* Advanced clinical assessment skills, with the ability to recognise and respond to acute and chronic medical conditions.
* Proficient in interpreting ECGs, echocardiograms, and other diagnostic tools
* Effective IT skills including use of M365 applications, including but not limited to Excel, PowerPoint, Outlook and Word
* Ability to work autonomously
* Excellent, time management, organisational skills and the ability to work autonomously and as part of a team
* Strong effective communication and interpersonal skills, with the ability to build rapport and trust with patients and multidisciplinary teams
* Knowledge of current guidelines, local pathways, and best practices in community care

* Use of Outcomes Tools

* Extensive Clinical experience, particularly in the community or outpatient setting
* Experience of leading a team
* Evidence of involvement in clinical audit
