Job Overview
The Mental Health Practitioners have a crucial role in processing, assessing, and determining the mental health and social care needs of referred individuals. They will assess ongoing needs and facilitate service provision. The role requires excellent risk assessment and management skills, including positive risk taking and decision making, to enable safe and effective responses for a largely telephone/assessment-based client group.
Participants will contribute to the delivery of the service's aims and objectives, identifying factors that may inhibit success. They will assist in delivering high standards of mental health care and monitor the effectiveness of care systems.
Based either with the Shropshire or Telford Crisis Resolution Home Treatment Team, depending on preference, with occasional work at The Redwoods Centre or Hall Court. The role involves close collaboration with the Crisis team and Access Call handlers.
The post is for 19 hours per week, alternating weekly between:
Saturday and Sunday, 8am - 6:30pm, with a 45-minute break
Main duties include:
1. Leading assessments for individuals referred to the Access Team.
2. Developing and maintaining effective communication and positive working relationships with colleagues, departments, and other agencies.
3. Organising and delivering informal, team-based training focused on the needs of clients with functional mental health issues across all ages.
4. Managing time effectively to optimise staff and resource use, acting as Shift Co-ordinator, and informing senior team members of issues affecting service delivery.
