The therapy team includes Physiotherapists, Occupational Therapists and Support Workers who work closely with the wider multi-disciplinary team to provide rehabilitation to patients in their own home.

Essential requirements include good communication skills, ability to plan and organise, and evidence‑based care. You must have access to a vehicle for work purposes or be able to travel as required.
Main duties of the job
Our Integrated Community Services Teams are enthusiastic, with a breadth of experience, and share a goal to offer the highest standard of care.
Team members are expected to carry their own clinical caseload, attend regular therapy meetings, and undertake individual and joint therapy assessments and treatment sessions with MDT members where appropriate. Previous post qualification experience is desirable but not essential.
Rehabilitation is delivered to patients in their own homes with focus on improving health and well‑being, working toward patient‑centred goals, preventing hospital admission, or facilitating discharge home.
You will develop knowledge and experience around Falls and Frailty and Triage.

Qualifications
* Diploma or degree in Physiotherapy
* Registered with HCPC
* Evidence of CPD
