Randstad is recruiting a Social Worker to join the Home First Hospital Team on a temporary basis. This role involves working across local hospitals and community settings to support patients and ensure smooth transitions of care.
About the Role:
* Work primarily in hospitals, with community visits as required
* Collaborate with healthcare teams to provide timely social work support
* Manage caseloads and complete assessments in line with best practice
Requirements:
* Qualified Social Worker
* Valid UK driving license and access to your own car
* Ability to work across multiple sites efficiently
