Surrey Downs Health & Care We are seeking a dynamic and motivated Band 7 Physiotherapist (or an experienced Band 6 looking to progress) with strong assessment and treatment skills in stroke and neurological rehabilitation to join our Stroke Therapies Service. This is an exciting opportunity to work within a supportive multidisciplinary team, delivering high-quality, patient-centred care. You will play a key role in the rehabilitation journey of patients with complex neurological needs, contributing to innovative service development and clinical excellence.

* Demonstrates a willingness to participate in the emergency on-call and weekend respiratory physiotherapy service, working autonomously when undertaking these duties
* Excellent clinical reasoning and decision-making skills

Responsibilities
* Provide a high standard physiotherapy service to stroke and neurologically impaired adult inpatients.
* Undertake advanced assessment and treatment of stroke and neurologically impaired and other in-patients, determining clinical diagnosis and treatment indicated and maintaining associated records as an autonomous practitioner.
* Provide highly specialised advice to patients, family, carers, and health and social care professionals regarding the management of patients with highly complex neurological conditions.
* Interpret and analyse clinical and non-clinical facts to provide accurate recommendations on further needs and development of comprehensive discharge plans.
* Take a lead role in the case management of patients with highly complex neurological disabilities who require onward referral to specialist Neuro-rehabilitation and, where appropriate, facilitate transfer to appropriate interim facilities.
* Communicate with the MDT and the patient/family regarding the status of rehabilitation referrals and requirements for acceptance/transfer, highlighting requirements to support referrals (completion of reports, nutritional management e.g. insertion of PEG, setting and implementation of MDT goals) to the MDT.
