Job Overview: Help patients get home safely by keeping our wards moving. The role involves coordinating patient flow and discharge processes, working closely with nurses, therapists, social workers, and multidisciplinary team members to ensure smooth transitions and accurate record‑keeping.
Key Responsibilities:
* Work closely with nurses, therapists, social workers and other multidisciplinary team members to coordinate patient flow and ensure timely discharge.
* Maintain accurate records, complete referrals, and support clear communication across services.
* Build positive relationships with patients and their families on the wards, helping them understand the discharge process and keeping them informed throughout their stay.
Location: Paulton Memorial Hospital is located in the heart of the village of Paulton, in the Somerset countryside close to the Mendip Hills, with good transport links to Bath, Bristol and surrounding towns.
