Hours: 37.5 hours per week, Monday to Friday

Main duties of the job
Cumbria Health delivers high-quality, responsive health services across the county. We are seeking motivated Health and Work Coaches to support the WorkWell programme in Barrow.
As a Health and Work Coach, you will assess and provide specialist employment advice, support, and interventions. Responsibilities include managing a caseload, developing individual Thrive in Work and Return to Work plans, and identifying employment barriers. You will guide participants in overcoming these barriers, signpost them to relevant services, and help them achieve their employment goals.
The WorkWell service supports those facing challenges entering or sustaining employment, including participants on long-term sick leave or dealing with health conditions. Using holistic, light-touch therapy and health assessments, WorkWell provides early intervention to improve employment outcomes.
Additionally, the service advises employers on reasonable adjustments and employee retention, supporting a healthier, more inclusive workforce. This role contributes to reducing health inequalities, as employment is a vital factor in maintaining good health and well-being.

Job responsibilities
1. Manage a caseload of participants who have experienced common mental, physical, and social health problems or long-term conditions.
2. Carry out vocational assessments and develop Strive to Work and Thrive to Work plans with WorkWell participants.
3. Ensure participants are eligible to attend the service.
4. Provide an evidence-based, person-centered, and holistic initial assessment of barriers.
5. Provide light-touch holistic support for health-related barriers to employment.
6. Provide pathways into local services to address specific needs and ensure referral pathways into and from WorkWell services to maximize engagement opportunities.
7. Maintain accurate case notes, action plans, and participant paperwork, uploading these to the management information system within the specified timeframe.
8. Provide information, advice, guidance, and support regarding pathways to gain, return to, and sustain work.
9. Support individuals in obtaining benefits advice, completing paid job applications, creating CVs, and preparing for interviews.
10. Liaise with and arrange meetings with health professionals, partner agencies, and/or employers concerning individual needs to increase positive outcomes.
11. Deliver training and workshops on job applications, CV writing, and interview skills.
12. Participate in informal training with external organizations to raise awareness of barriers to employment.
13. Monitor progress and outcomes, ensuring deadlines are met and KPI evidence is collected accurately.
14. Work flexible hours as required to meet service needs.
