We have an exciting opportunity to join our Health Visiting Service as an Early Help Family Practitioner!
Working alongside Health Visitors within our Health Visiting service, you will support the delivery of the Healthy Child Programme (0-5) to achieve positive outcomes for children and families. This includes delivery of specific elements of the programme and a focus on the high impact areas across Bath.

As travel is required for this role, a valid UK driving license and access to a vehicle for work purposes are essential.
Main Responsibilities
As an Early Help Family Practitioner, your main responsibilities will include:
1. Carrying out individual work with families as delegated by an accountable Health Visitor and providing feedback on outcomes.
2. Supporting the facilitation of local healthy family drop-in hubs.
3. Planning and delivering universal 3-4 month health promotion sessions, facilitating discussions with groups of parents/carers.
4. Undertaking universal mandated reviews, including 6-8 week, 10 month, and 24 month reviews, and referring families requiring further assessment to the Health Visitor.
5. Carrying out maternal mental health screening at 6-week and 10-month reviews using evidence-based tools.
6. Participating in CO monitoring at key mandated contacts as per the pathway.

The Ideal Candidate
The ideal candidate will possess an NNEB qualification or equivalent, such as a BTEC (National Diploma in Nursery Nursing), NVQ level 3 in Childcare and Education, or a Foundation Degree in Early Years/Education.
A Full UK driving license and access to a vehicle are essential.
We are looking for someone with good communication and interpersonal skills, who can work flexibly and adapt to service needs.
The successful candidate will have a solid understanding of child development, parenting, and the ability to apply behavioral programs.
