What you will do:
* Conduct statutory reviews for citizens residing in care home placements.
* Collaborate with health partners to facilitate status changes for care homes, including Continuing Health Care.
* Serve as a contact point for care home managers, providing support to social work colleagues during unplanned reviews.
* Support provider performance, including managing processes for care home closures to aid citizens and families effectively.
* Maintain a consistent approach towards providers, working in tandem with Contracts and Commissioning colleagues to uphold best practices.
* Actively engage in benchmarking and knowledge development in dementia care, promoting best practices across the service.
