🌟 About the Role:
Join our dedicated Older People's Social Work Team as a Social Worker, supporting adults across East Herts, Broxbourne, and Welwyn Hatfield. This 6-month contract is funded through winter pressures, offering a valuable opportunity to make a meaningful impact during a critical time.
🔧 Key Responsibilities:
📍 Based in the locality, supporting people in their own communities
🏥 Help reduce hospital admissions through timely intervention
📋 Complete Care Act assessments and reviews
🧩 Arrange and coordinate appropriate care and support packages
🛡️ Act as Safeguarding Enquiry Officer
🧠 Undertake Mental Capacity Assessments and Best Interest decisions
🤝 Work collaboratively with professionals across health and social care
✅ What We're Looking For:
🎓 Qualified Social Worker
🧾 Registered with Social Work England
🚗 Full UK driving licence
📊 Strong organisational, communication, and IT skills
👵 Experience working with older adults, including those with cognitive impairments
