Job Overview
Post holder will work as part of the nursing team, delivering evidence based high quality clinical care that focuses on the direct needs of the patient.
Responsibilities
* Assist registered nurses/practitioners in the provision of delegated care of patients.
* Provide holistic needs for the individual in conjunction with other disciplines within the health care environment.
* Undertake a broad range of duties and work independently and as part of the team, supervising patients and assisting them with future care plans.
* Offer assistance in induction of junior healthcare workers and apprenticeships.
* Deliver elements of health and social care, having the knowledge and assessed level of competence locally assessed.
* Provide day‑to‑day care for service users under the supervision of a clinician, adhering to best practice, standards and guidelines, and contributing towards the service users care and treatment.
* Work in partnership with service users, other members of the MDT staff as appropriate, to achieve optimum health outcomes for clients.
* Provide a high and consistent level of holistic care, maintaining the dignity and equality of the individual and providing direct care.
Job Duties
An opportunity has arisen for a caring and enthusiastic individual to work full time as a Band 3 Health Care Support Worker within the Denbighshire Community Mental Health Team.
Candidates must hold a positive attitude to the mental health care of the older persons client group and ensure that the individual's needs for privacy, respect, dignity and confidentiality are maintained at all times.
The successful candidate will provide high standards of care and work as part of a well‑established and supportive multidisciplinary team based at Community Mental Health Team in Denbighshire.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Qualifications
* Essential Criteria:
o QCF Level 3 or equivalent qualifications/experience.
o NVQ Level 3 in Health & Social care related subjects or equivalent qualifications/experience.
o Good knowledge of anatomy & physiology.
o Understanding of multi‑disciplinary/agency working and of individual team roles.
* Desirable Criteria:
o All Wales HCSW induction framework.
o Training in Mental Health.
o Understanding of risk management and health and safety issues.
Experience
* Essential Criteria:
o Experience of working as a HCSW without direct supervision in a clinical setting and being managed by a qualified practitioner.
o Good understanding of risk assessment and management.
o Significant experience of working in a caring environment for older people.
* Desirable Criteria:
o Experience of working in a mental health care setting.
o Experience of working in the community.
Skills
* Essential Criteria:
o Evidence of ability to provide wound care using aseptic technique.
o ANNT.
o Phlebotomy.
o Basic observations.
o Ability to communicate clearly and accurately.
o Ability to be adaptable to new and changing circumstances.
o Ability to work alone under supervision and to understand when to seek clinical advice and support.
* Desirable Criteria:
o Vital signs recording (BP, pulse, temp, respirations, blood sugar).
o Venepuncture.
o Routine ECG recording.
o Urine testing.
o Any other specific clinical skills at this level of practice pertaining to role as per service need.
o Knowledge and understanding of clinic activities.
