Crescent Care’s Housing & Homelessness Assistance program is seeking a dedicated Case Worker to provide end-to-end support to individuals and families at risk of homelessness. You will work closely with clients to develop sustainable housing solutions and improve long-term outcomes.
Key Responsibilities:
* Conduct housing assessments and develop case management plans.
* Provide tenancy support, budgeting advice, and assistance securing long-term accommodation.
* Advocate on behalf of clients with housing providers and government agencies.
* Facilitate access to mental health, employment, and financial counselling services.
* Prepare detailed case notes and compliance reports.
Requirements:
* Diploma or Degree in Community Services, Social Work, or related field.
* Strong knowledge of the housing and homelessness sector.
* Experience supporting clients with complex needs.
* Excellent interpersonal and negotiation skills.
* Current driver’s licence and reliable transport.
