Overview
COMMUNITY ASSISTANT PRACTITIONER: An exciting opportunity to join the Mildenhall INT within the West Suffolk Community. This role involves assessing and providing therapy under the direction of an occupational therapist and a physiotherapist. The post holder will be based within the Mildenhall INT (with a satellite base at Brandon) and may be required to work within the Newmarket INT at times. The role involves working autonomously within a community environment, while also collaborating as part of a multi-disciplinary team.
The post holder will hold a patient caseload and deliver therapy, progressing to the level of undertaking basic assessments autonomously with support. Establishing rapport with people receiving therapy and working closely with health and social care colleagues, carers, family members and other professionals is important. Collaboration with the INT Nursing and social care team is essential to ensure optimised care goals for patients in the locality.
Responsibilities
* Assess and provide therapy under the direction of an occupational therapist and a physiotherapist.
* Work autonomously within a community environment and as part of a multi-disciplinary team.
* Hold a patient caseload and deliver therapy, progressing to undertake basic assessments autonomously with support.
* Establish rapport with people receiving therapy and work with health and social care colleagues, carers, family members and other professionals.
* Collaborate with the INT Nursing and social care team to optimise care goals.
* Deliver high-quality, efficient therapy services focused on improving independence and self-management using personalised care.
* Work independently under the supervision of therapists, carrying out assessments and reviewing progress.
* Participate in regular supervision, development opportunities, annual appraisals, and training.
Qualifications and Skills
* Excellent communication and interpersonal skills with a positive attitude to self-directed learning and practice.
* Previous experience in a caring profession is beneficial; no previous therapy experience is required.
