Support to manage activities of daily living
Preferred days/times:
Tuesdays - 3.5 hours per week
Wednesdays - 3.5 hours per week
Thursdays - 3 hours per week
Total - 10 hours per week
Times to be arranged with PA
Tasks/duties:
Tasks included supporting with meal and fluids prep, using the toilet and getting dressed, minor household tasks, and going out and about in the community
Client main diagnosis:
Fibromyalgia
Fatigue
Chronic widespread pain in joints
Costochondritis
Hyperhidrosis (excessive sweating),
Coccyx removed in 2016
Plantar Fasciitis
Stomach and bowel issues including IBS
Hiatus Hernia
Depression
Anxiety
Short term memory issues
Speech impairment
Previous hip surgery
Tendonitis and bursitis of the shoulder causing reduced mobility in right shoulder
Patella tendonitis in knees (give way both sides)
Osteoarthritis
Adenomyosis and Border line personality disorder.
Manual Handling
Moving and handling support required - help getting up and down as needed
Equipment in place:
Shower stool
Handrails
Crutches
Wheelchair
Electric scooter
The client's speech is generally clear and understandable however they are unsure whether their medication impacts at times when they struggle to find words, which is heightened when anxious or stressed. The client has been diagnosed with speech discrimination whereby although their hearing is fine (tested), they struggle to recognise some spoken words. They have no issues with written communication.
