Following the success of the development of this new role within the Home Care and Reablement Service an exciting opportunity has arisen for us to expand our exisiting workforce.
We are looking for an extremely motivated and experienced individual to work as part of a dedicated team to contribute to the assessment and coordination of care and support that offers individuals the provision of personal and practical assistance, enabling them to continue to be supported in their home environment.
We are looking for highly motivated, dynamic people with a wide range of skills who are reliable and hardworking and can work as part of a team and are able to adapt to change in a fast moving, innovative and customer focused service.
Your role will be to help vulnerable people maintain and regain their independence and remain in their own home with an improved quality of life. The role also involves supporting individuals upon discharge from hospital as part of the Discharge to Assess pathways and contribute to the assessment of individuals needs and planning as part of the a Trusted Assessor (Level 3) function. (Assessing, provision and fitting of low level equipment)
The successful candidates will be working on a rota, including weekends, bank holidays and shift working.
