Job Overview
The role is to work closely with clinical and safeguarding teams to support all activities necessary to ensure the Trust meets its responsibilities to patients with dementia and learning disability. You will promote best practice, implement standards and monitor processes in line with national and local multi‑agency guidance. The position requires working independently, providing expert advice and support to a variety of staff on complex decisions around learning disability, autism and dementia.
Key Responsibilities
* Deliver safe, effective, patient‑centred care and provide assurance of quality in line with the Trust’s core objectives.
* Develop and deliver continuous improvement initiatives to meet quality indicators, KPIs and safety standards.
* Plan and deliver education activities for staff in accordance with the learning disability and autism code of practice.
* Review and improve patient pathways to enhance quality of care and efficiency.
* Lead clinical practice standards and strategies for the delivery of excellent care for people with learning disability, autism or dementia.

Person Specification
Experience
* Registered Nurse – preferably Learning Disability registered or trained.
* Experience of multidisciplinary working.
Skills
* Strong communication skills across multidisciplinary teams.
* Experience of managing complexity in care pathways.
* Desirable: counselling skills.
Knowledge
* Significant relevant clinical experience with learning disability or dementia.
* Experience of developing others through education, supervision and training.
* Knowledge of relevant legislation: Care Act 2014, Mental Capacity Act 2005, Deprivation of Liberty Safeguards 2009 / Liberty Protection Safeguards.
Applicants must hold appropriate UK professional registration, be eligible to work in the UK, and have completed a satisfactory DBS disclosure.
