Job overview
Works as part of an integrated multi-disciplinary team (MDT) including clinicians, therapists, social care, and community services to establish and oversee discharge plans that are safe, timely, and efficient whether patients are heading home or transitioning to continued care settings.
Main duties of the job
* Conduct holistic assessments to determine a patient's health and social care needs
* Assist in the case management of complex discharge planning
* Coordinate Discharge to Assess pathways and integrate Home First as the preferred outcome when appropriate
* Liaise with patients, families and external agencies (e.g. community, voluntary, commissioning services)
* Initiate referrals, manage caseload and problem-solve to avoid discharge delays
* Coordinate the Transfer of Care Hub, provide advice to ward staff and other professionals, including educational support
* Supervise ward discharge coordinators, discharge facilitators and admin staff
