Joint Community Rehabilitation Service (JCR) – Lewes & Havens. Full Time, Permanent.
About the Service
The Joint Community Rehabilitation Service provides a short‑term, 7‑day service that assesses and treats patients in their own homes or at our Community Rehabilitation Hub. The service aims to prevent unnecessary hospital admissions, enable earlier discharges, and promote independence and quality of life.
Key Responsibilities
* Assess and treat patients in their homes or at the hub.
* Travel around the county to provide timely care.
* Work within a multidisciplinary team of Physiotherapists, Occupational Therapists, and Support Workers.
* Participate in supervision and training opportunities for professional development.
* Cover weekends on a rota basis (approximately one weekend day every four weeks).
