JOB DETAILS JOB TITLE: Clinical Lead Mental Health Nurse BAND: Band 7 Pay Rate £25 - £35 per hour HOURS: 37.5 hours (unless otherwise stated) Duration: 12 weeks DEPARTMENT: Inclusion LOCATION: Redditch REPORTS TO: Team Manager ACCOUNTABLE TO: Senior Cluster Manager / Head of Operations JOB PURPOSE Provide expert clinical knowledge and interventions, advice, support and supervision in the multi- disciplinary pathway, to colleagues, service users and carers. The post holder will be responsible for providing leadership to deliver evidence based care and interventions and providing professional nursing leadership within the pathway team. RESPONSIBLE FOR:  To lead the team in expert assessment, planning, implementation and evaluation of care for a specific group of service users.  To supervise and teach service users and carers, professionals from the multi-disciplinary teams, qualified and unqualified staff and the general public  To work in partnership with service users, carers and other agencies to ensure effectiveness of outcomes.  To develop, with specialists, non-medical prescribing clinics within the team. KEY RESPONSIBILITIES Leadership 1. To provide appropriate advice to team staff, multi-disciplinary and Management Groups. 2. To monitor and evaluate practice based on contemporary and current trends and initiate changes in the light of these findings. 3. To ensure that all statutory and professional responsibilities of the NMC Codes of Conduct are met alongside other professional related bodies 4. To promote, encourage and develop integration between Prison and Community services. 5. Facilitate effective recovery focussed care. 6. Encourage and facilitate multi-disciplinary and multi-agency working. 7. Provide advice, information and support in relation to Complex Mental Health Care. 8. In conjunction with the Team Manager, monitor and manage the performance of the team and individual team members, to ensure achievements of Trust organisational targets/objectives. 9. Undertake operational management supervision and ensure appropriate clinical / professional supervision arrangements are in place for nursing staff. 10. Ensure compliance with the Trust's objectives in relation to the Governance Agenda, achieving a continuous process of monitoring and improving safe effective care delivery 11. Encourage staff to share good practice, develop ideas to improve services and promote research. 12. Maintain a strong overview on environmental standards and address any issues relating to safety, dignity, poor quality and poor hygiene and Infection Control. 13. Promote service audit, evaluation and research. 14. Ensure robust risk assessment and management accordance with policies. 15. In partnership with the Team Manager, ensure that all nursing staff receive a Performance development conversations PDC appraisal and have an appropriate personal development plan 16. Undertake mandatory training and have an appropriate personal development plan in place. Quality 17. Within the area of clinical specialism; to ensure the maintenance and monitoring of service standards. 18. Collect and collate data/ information effectively for the purpose of audit and service performance. 19. To contribute towards research in the area of expertise 20. To identify and evaluate, with others, areas for potential service improvement and agree further action 21. To lead the development of service standards and policy, including benchmarking, and identifies areas for potential service improvement. 22. To contribute to the maintenance and monitoring of service standards. 23. To promote shared decision making with Service User involvement in all aspects of their care. 24. To contribute and make suggestions to the improvement of services. 25. To identify issues of concern at work and alerting appropriate personnel 26. To present a positive image of the team, service and trust. 27. To work effectively in own team and as part of the whole organisation. 28. To ensure the availability of clinical supervision and preceptorship 29. To ensure structures are in place for effective appraisal, personal development and performance management in conjunction with the pathway manager and MDT team. 30. To assess and plan care in a manner that is consistent with the evidence based practice, policies and procedures and the management of risk. Clinical 31. To act/train as a supplementary nurse prescriber to the team or work to ensure supplementary prescribing can be undertaken. 32. To act as an autonomous practitioner in requesting the input from other professionals and pathways 33. To act as the pathway link, enabling other pathways to engage support for a service user. 34. To utilise expert clinical knowledge and nursing skills and /or specific to discipline to provide assessment, intervention and clinical guidance as appropriate. 35. To demonstrate knowledge and skills within the defined area of expertise 36. To work with MDTs in management of care and the respond to changes in service users' health. 37. To act in a manner to ensure that customs, individuality, values, sexuality and spiritual beliefs, are actively respected and that service users are supported to fulfil these. 38. To identify and provide intervention and emotional support for service users. 39. To create, develop and maintain professional supportive relationships with all members of staff with other professionals and agencies to enhance recovery. Risk Assessment 40. To understand the range of risks potentially faced by the service user group 41. To work in partnership with others to identify and assess the nature, location and seriousness of the particular risks and to advise on appropriate action. 42. To recognise potential crisis situations and act responsively and responsibly, to resolve the situation. To lead on appropriate use and contribution to the ACCT process. 43. To contribute to the CPA process and support service users, and others, through this. 44. To be aware of the potential for vulnerability and abuse amongst the service users/families and use the local policy to inform appropriate personnel in line with POVA and Child Protection and MPFT Policies. 45. To work with other members of the multi-agency team to plan, monitor and review the effectiveness of risk management plans 46. To take all possible precautions to safeguard the welfare and safety of staff, service users, visitors and the public, by implementing all policies related to health, safety, security and risk. 47. To coordinate risk management plans relating to safeguarding adults and children and maintain an accurate record of risk, actions taken and investigations. Professional / Personal Development 48. To receive clinical supervision and participate in appraisal and performance development. 49. To adhere to the Nursing and Midwifery Council code of professional conduct for nurses and other professional bodies dependant on the role of worker in the pathway 50. To ensure attendance at all Mandatory and statutory training. 51. To identify development needs and set own personal development objectives in discussion with reviewer. 52. To work towards achieving competencies outline within role and the collection of evidence within personal portfolio. 53. To be responsible for the orientation and induction of all staff new to the service. 54. To contribute to the experience of learners and to ensure delivery of mentorship requirements meeting NMC standards for nursing students. 55. To develop an environment that is conducive to learning, promoting staff development and involvement 56. To plan, deliver and evaluate programmes of education within the specialism, informed by learners 57. To participate in the trust professional development strategy. Systems and equipment 58. Required to use VDU equipment for substantial periods of time 59. Required to input into and interrogate the electronic trust health record. 60. Required to extract and utilise information from performance "Dashboard" reports 61. In conjunction with the Team Manager, input and extract information from ESR and E-Rostering. Decisions and judgements 62. Provide managerial and clinical supervision to nursing staff in the team 63. Provide clinical supervision to other team members 64. Regularly undertake the assessment, planning and delivery of care for people experiencing complex mental health related difficulties 65. Under the supervision of the psychiatrist regularly provide prescribing advice and provide prescriptions as per agreed formulary 66. Undertake NMC re-validation sign off for nurses in the team 67. Lead the development, implementation and evaluation of clinical processes within the team and contribute to the development of clinical process across pathways. Communication and Working Relationships 68. Communicate policies and practical procedures within the team and externally to ensure effective understanding and implementation. 69. This will involve regular contact and exchange of complex, confidential and sensitive information between staff, service users, carers, general public and internal/external agencies, where tact and diplomacy is required in a form that is appropriate to them and the situation and constructively manages barriers to effective communication. 70. Maintain a position of integrity and confidentiality at all times, communicating effectively and keeping accurate and complete records that are consistent with relevant legislation, policies and procedures. 71. To be responsible for the recording of all contacts with service users in electronic records / case notes in line with MPFT record keeping policy. 72. To ensure that all communication takes place in a manner that is consistent with legislation, policies and procedures. 73. To develop measures to improve communications where barriers exist. 74. To ensure that consent to intervention or assessment is sought in a manner that is meaningful to the service user. 75. To communicate with people in a manner that is consistent with their level of understanding, culture and background. Physical demands of the job 76. Keyboard skills 77. Prison Environment 78. The post holder will have the ability to travel across trust sites. 79. There is a frequent requirement for sitting in a restricted position for a substantial proportion of the working time. Most challenging/difficult parts of the job 80. Frequent concentration is required when dealing with performance reports and staffing issues. 81. Regularly required to provide assessment and interventions for service users experiencing complex mental related difficulties. 82. Work is often interrupted throughout the day to deal with queries. 83. Regular exposure to distressing or emotional circumstance when dealing with 84. Service users either face to face or on the telephone. 85. Regularly deals with staffing issues and service user/carer complaints. 86. Exposure to a VDU is frequent in line with H&S guidelines. 87. Exposure to verbally abusive language is occasional. 88. The post-holder will need to travel across bases to visit service users attend meetings; therefore a driving license is required. Infection Control Maintain an up to date awareness of the infection control precautions relevant to your area of work and implement these in practice. As a minimum, this must include hand hygiene, the use of personal protective equipment, the use and disposal of sharps and communicating the importance to patients, prison staff and other health care staff you are working with. Details of the precautions and sources of advice and support to assess and manage infection control risks are provided through mandatory training which all staff must attend at intervals defined in the Trust policy on mandatory training and can be found in the Trust's infection control policies and national guidance, such as that published by NICE.
