Responsibilities
* To manage referrals into our service and provide evidence-based care to support patients in their own home.
* To work alongside general practice, therapy and Home visiting colleagues.
* To assess, plan, implement and evaluate individualised patient care, promoting self-care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity.
* To provide advice and support to junior staff in the delivery of high quality care and actively support the community nursing sister/charge nurse in the management of the health care environment.

Qualifications and Experience
Essential
* RGN
* Evidence of recent relevant training
Experience and Knowledge
* Experience of working in the community
* Evidence of research-based/reflective practice
Other
* Ability to travel across Derbyshire County visiting patients at home and across the county for meetings and training
* Able to work weekends and bank holidays
