About the Practice/Department/Team
Lakeside St Neots is in a lovely town location with around 21,000 patients. We are a diverse and dynamic team of GPs, Pharmacists, Physios, Social Prescribers, Practice Nurses, Health Care Assistants, Nursing Associates, Emergency Care Practitioners and ANPs.
Job Introduction
As a registered nurse with diabetes interest, you will provide and maintain a high standard of diabetes management for Type 1 & type 2 patients, which includes diagnosis, monitoring & development of individual management plans, agreeing these as appropriate with the patient & other health professionals. Specifically in Diabetes management and desirably in CHD and HF; you will work strictly in accordance with specific practice guidelines & protocols. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy, and performance.
What you will do
(Main duties -list not exhaustive)
* Responsible for providing and delivering primary health care interventions and care effectively and efficiently.
* Working collaboratively with the multi-disciplinary general practice team and other healthcare professionals to promote integrated pathways of care to meet patient needs.
* Effective communication with patients, carers, and all members of the MDT
* Effective prioritisation and completion of daily work in line with clinical / patient need and practice priorities.
* Maintenance of accurate and legible patient records in line with professional and organisational requirements – this includes contemporary paper records and electronic patient records system.
* Actively participates and contributes to nurse team meetings.
* Initiates health promotion activities with individuals, patients, and carers
* Participates in clinical audit and the implementation of changes to ways of working and/or clinical practice because of findings, recommendations, and action plans.
* Proactive in improving and influencing the patient care pathway/ journey.
* Participates in risk assessment and promotes the safety and well-being of patients, staff, and any other persons always and assist in ensuring a safe work environment.
* Ensure that any complaints or incidents are identified and reported to the lead GP, Nurse Manager or Practice Manager in accordance with the organisational policy and expected professional practice.
* Monitors and orders stock levels and equipment in line with budget and lean principles.
* Responsible for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained.
* Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
Duties include: -
* Working independently as a registered nurse with diabetes interest in type 1 & type 2 management. Including managing diabetic reviews and taking appropriate action as per current guidelines
* Assisting the general practice team with monitoring, educating, and reviewing patients on the diabetic register
* Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary.
* Recognise health promotion opportunities and give brief, focussed lifestyle advice using motivational interviewing techniques which may include weight loss, diet, exercise, smoking cessation
* Assist with nurse-run diabetic clinics and carry out clinical measurements and near patient testing in line with NICE guidance and Quality and Outcome Framework indicators.
* Recognise signs and symptoms that may indicate undiagnosed conditions that may exist alongside diabetes.
* Work in accordance with Practice protocols to ensure the safe and effective management of patients with Type 1 or Type 2 Diabetes
Essential Criteria
* Registered Nurse
* NMC 1st level Registration
* Evidence of continued professional development
* Accredited Diploma Level training or equivalent in diabetes management Type 1 & Type 2.
* Experience in the management of Type 1 & Type 2 diabetic conditions in primary or secondary care.
* Willingness to undertake training and education to achieve core skills and experience required of advanced diabetic management.
* Experience of managing/dealing with the public/patients
* Willingness to improve and develop the patient experience and patient care pathway
* Experience of working within healthcare governance systems and understands role and responsibilities for CQC and within risk management
* Experience of using clinical systems & Microsoft Office software
* Understanding and application of the principles of Infection Prevention & Control
* Excellent record keeping and effective interpersonal communication skills
* Can demonstrate skills in problem solving and decision making
* Ability to prioritise, delegate and organise workload, meeting and achieving deadlines
* Maintains professional portfolio consistent with NMC revalidation program; evidence of being a reflective practitioner.
* Is motivated, committed and enthusiastic in all aspects of professional practice
* Self-aware, can recognize own limitations
* Can use own judgement, resourcefulness and common sense
* Ability to be adaptive and responds well to stressful situations
