Due to an upcoming retirement, we are looking to welcome an experienced Advanced Nurse Practitioner (ANP) to join our dynamic and supportive team at Whitwick Road Surgery.
Main duties of the job
We're looking for a highly motivated, experienced clinician with independent prescribing rights and a strong background in primary care. The successful candidate will work autonomously within their scope of practice, managing minor illnesses and long-term conditions, and contributing to the smooth daily running of the surgery.
Key Requirements:
* Registered Nurse Practitioner with relevant qualifications
* Independent prescriber (V300)
* Previous experience of working in general practice / primary care is essential
* Able to manage acute and chronic conditions, conduct assessments, and make referrals as appropriate
* Excellent communication and teamwork skills

Job responsibilities
The following are the core responsibilities ofthe Advanced Nurse Practitioner. There may be, on occasion, a requirement tocarry out other tasks; this will be dependent upon factors such as workload andstaffing levels:
Clinical Practice
Assess, diagnose, plan, implement and evaluatetreatment / interventions and care for patients presenting with anundifferentiated diagnosis.
Clinically examine and assess patient needsfrom a physiological and psychological perspective, and plan clinical careaccordingly.
Assess, diagnose, plan, implement and evaluateinterventions / treatments for patients with complex needs.
Proactively identify, diagnose and managetreatment plans for patients at risk of developing a long-term condition (asappropriate).
Diagnose and manage both acute and chronicconditions, integrating both drug and non-drug-based treatment methods into amanagement plan.
Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice.
Work with patients to support compliance with,and adherence to, prescribed treatments.Provide information and advice on prescribed or over-the-countermedication on medication regimens, side-effects and interactions.
Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care.
Support patients to adopt health promotionstrategies that promote healthy lifestyles and apply principles of self-care.
Communication
Utilise and demonstrate sensitive communicationstyles, to ensure patients are fully informed and consent to treatment.
Communicate with and support patients who arereceiving bad news.
Communicate effectively with patients andcarers, recognising the need for alternative methods of communication toovercome different levels of understanding, cultural background and preferredways of communicating.
Anticipate barriers to communication and takeaction to improve communication.
Maintain effective communication within thepractice environment and with external stakeholders.
Act as an advocate for patients and colleagues.
Ensure awareness of sources of support andguidance (e.g. PALS) and provide information in an acceptable format to allpatients, recognising any difficulties and referring where appropriate.

Produce accurate, contemporaneous and completerecords of patient consultations, consistent with legislation, policies andprocedures.

Assess effectiveness of care delivery throughself and peer review, benchmarking and formal evaluation.

In partnership with other clinical teams,collaborate on improving the quality of health care responding to local andnational policies and initiatives as appropriate.

Support and participate in shared learningacross the practice and wider organisation.
Use a structured framework (e.g. root-causeanalysis) to manage, review and identify learning from patient complaints,clinical incidents and near-miss events.
Assess the impact of policy implementation oncare delivery.
Understand and apply legal issues that supportthe identification of vulnerable and abused children and adults and be aware ofstatutory child/vulnerable patients health procedures and local guidance.
