Work according to the Nursing and Midwifery Council (NMC) Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker, taking responsibility for the assessment of care and health education needs.
Develop, implement, and evaluate care plans in conjunction with service users, carers, and other professionals, ensuring individual needs are met.
As named nurse, undertake mental and behavioural state examinations for own service users and new admissions, conducting these at least weekly, along with clinical risk assessments and management plans.
Assist service users in meeting personal care needs, such as bathing and washing, as outlined in the Care Plan. Delegation of these interventions is appropriate where applicable.
Liaise with and provide specialist assessment and advice to other agencies and professionals, including participation in Multi-Agency meetings to facilitate ongoing treatment and care, ensuring adherence to National Service Framework (NSF) guidelines.
Support service users in managing their medication within a concordance framework and relevant policies. Administer medications, including intra-muscular injections and depot medication as appropriate, while monitoring for side effects. Delegate medication checking and delivery when suitable.
