What You'll Do
As a Care Coordinator, you will be at the heart of our operations, ensuring seamless, high-quality care delivery. Your key responsibilities will include:
* Coordinating staff rotas and managing visit schedules to ensure continuity of care.
* Liaising effectively with clients, their families, and other healthcare professionals to ensure integrated support.
* Monitoring care quality, maintaining accurate records, and ensuring CQC compliance across all services.
* Providing essential support and guidance to our dedicated care team, fostering their development and problem-solving
* Participating in the on-call rota (shared between 4 people) and responding to urgent situations to maintain service integrity.
What You'll Bring
To excel in this role, you will need:
* A minimum of 1-2 years of hands-on experience in a care setting, ideally within a community-based domiciliary care environment.
* A minimum of 1 year care coordinator experience
* A Level 3 qualification in Health and Social Care, or a demonstrable commitment to achieving this qualification
* Exceptional organisational, communication (verbal and written), and problem-solving skills.
* Proficiency in using digital systems for record management and scheduling.
* A full UK driving licence and access to your own reliable vehicle for community travel.
* A proactive, empathetic, and professional approach, with the ability to work effectively under pressure.
