CLINICAL CASE MANAGER

We are looking for experienced Clinical Case Managers or people with clinical experience in neurology and rehabilitation; Occupational Therapist, Physiotherapist, Nurse, Speech and Language Therapist or Social Worker. The role is diverse, hugely rewarding and stimulating. We really make a positive difference to our clients and their families.

This is a permanent full-time position – 37.5 hours per week, Monday to Friday.

Salary: Dependent upon experience - up to £44,250 however this is currently under review.

We are based in Wigan and the role offers some home-working. We see clients in their own homes across the North West of England.

The Role:

· You will be responsible for completing a comprehensive and holistic assessment of the client's needs, abilities and difficulties in relation to their desired goals.

-Complete and maintain a goal focussed Case Management Plan, risk assessment and Care Plan.

-Assemble and oversee a bespoke multi-disciplinary team of appropriate health-care professionals.

-Recruit and manage teams of support workers specific to the needs and wishes of the client.

-Produce detailed and accurate reports and effectively liaise with Solicitors who are involved in the client's litigation and / or from the Court of Protection.

-Maintain positive relationships with other professionals including those from health care, statutory services and legal sectors.

-Advocating for our clients with ambition; we don't settle for 'good enough' if we can aim for better.

-Continuous professional development; we provide excellent opportunities for this.

-Uphold the values of Active Case Management in your conduct with clients, colleagues and other professionals and support workers.

Requirements for this role:

* You will have a degree or diploma level qualification in Occupational Therapy, Speech and Language Therapy, Nursing, Social Work or Physiotherapy and be registered with the relevant governing body.

* Active Case Management will oversee your registration with the British Association of Brain Injury Case Managers and when eligible with the Institute of Registered Case Managers.

* Experience of working as a Clinical Case Manager is ideal.

* 2 years post-qualification experience in your profession is desirable.

* Experience of working in neurology / brain injury rehabilitation is desirable.

* It is essential that you have a full UK driving licence with use of a car and business insurance.

* Experience of working in a community setting is an advantage.

* You should speak and write clearly in English, with competency in writing reports and clinical documentation to a high standard.

* Experience / knowledge of the medicolegal sector is desirable.

* You will be motivated to see each client as an individual with unique experiences and goals.

* You must be reflective and honest, able to own your mistakes and learn from them.

* You will be a supportive team member, celebrating each other's successes and sharing tasks when required.

* You will be diligent and observant with an eye for detail and strong communication skills.

Job Types: Full-time, Permanent

Pay: Up to £44,250.00 per year
