Overview
Come and join Kent and Medway NHS and Social Care Partnership Trust as a Band 3 Support Time Recovery Worker at South Kent Coast Community Mental Health Team covering Dover, Deal and Folkestone.
Our service focuses on promoting recovery for people diagnosed with severe and enduring mental ill health. We provide comprehensive multi-disciplinary services and work closely with local specialist mental health teams, primary care, Kent Police, local statutory and non-statutory agencies, service users and carers. The post holder will support collaborative working with service users to promote engagement, independence and self-management.
The post holder must hold a full, valid UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Responsibilities
* Work closely with occupational therapists and nursing staff to support the effective functioning of a depot/clozapine clinic, including well-being checks.
* Visit people in their own homes as required.
* Complete Dialog Plus assessments by telephone or in person.
* Monitor and report on the mental state of service users.
* Provide direct support to service users and identify their needs.
* Develop and run recovery activities with Occupational Therapists where appropriate.
