Older Person Liaison Psychiatry Nurse

22.5 hours per week – two‑shifts per week. You will work a combination of long days and short days, seven days a week, between the hours of 08:00–20:00.
Mental Health Services in Rotherham have expanded through investments in 2019 and 2020, allowing us to grow our Hospital Liaison Mental Health Team. The Liaison Mental Health Service Model at Rotherham Foundation Hospital is part of NHS England’s “Achieving Better Access to 24/7 Urgent and Emergency Mental Health Care” guidance.
Older adults with mental health problems are three times more likely to attend an Emergency Department and five times more likely to be admitted to a general hospital as an emergency. Working within a Mental Health Liaison Team is therefore essential and rewarding.
We are looking for a highly skilled, autonomous Mental Health Nurse with strong clinical and risk‑management skills to join our Older Persons Hospital Liaison Team.
Your day‑to‑day role will support general hospital wards with older‑person mental health. You will conduct assessments, continue care and plan safe discharges.
Main duties of the job
* Respond to mental‑health crises in Emergency Departments and inpatient wards within one hour and on all urgent ward referrals within 24 hours.
* Carry out full biopsychosocial assessments and formulate treatment and collaborative care plans.
* Offer brief evidence‑based interventions as inpatient or short‑term outpatient follow‑up.
* Work within Rotherham General Hospital to reduce length of stay on general hospitals and improve follow‑up care.
* Provide advice and support to Rotherham General Hospital staff regarding mental‑health care for their patients.

Person Specification
Qualifications
* Registered Mental Health Nurse
* Post‑qualification in mental health
Experience
* Demonstrated post‑qualification experience in acute or community mental health services
* Experience working with older people’s mental health
* Demonstrated experience at a Band 6 level
Knowledge
* Understanding of working with older adults with mental health issues
* Experience managing older and/or adult mental‑health crises
* Knowledge of interactions between physical and mental health conditions
Skills
* Crisis management
* Conducting full bio‑social assessment
* Confidence in managing a high‑risk patient group
* Ability to work autonomously without direct supervision
* Feeding into an MDT approach to patient care

Salary
£38,682 to £46,580 per annum, pro‑rata
