From 1 December 2025 the pay range for this role will be £38,220 - £46,142 p.a. Band [9] (pro‑rata for part‑time colleagues). New starters begin at the bottom of the pay band, reflecting our commitment to equity and consistency. Responsibilities
* Work in the community with people aged 18 and over who have long‑term health conditions, physical disabilities, or mental health needs.
* Provide holistic assessments and interventions, including provision of equipment, minor or major adaptations, and Moving and Handling assessments.
* Facilitate greater independence and well‑being for people, families, and carers.
* Collaborate with a multidisciplinary team that includes qualified occupational therapists, social workers, ASYE social workers, and experienced non‑qualified social care staff.
