Job overview

Surrey Downs Health & Care are looking for a Band 7 Physiotherapist to be part of the SDHC Home First Epsom UCR team. Community experience, experience in ED & rapid responses are required for this post and it is a short term bank position over Winter; you will be onboarded to Bank and able to pick up further shifts following this line of work. Home visits for new rapids & follow ups are key duties. Hours are 34 per week in a pattern of three 12-hour shifts from 8am–8pm with a 40-minute break.


Main duties of the job

To undertake specialist physiotherapy and holistic assessment of patients as an autonomous practitioner, interpret results, use clinical reasoning and evidence-based practice, considering the individual’s clinical and psycho-social issues to determine treatment options and the best course of action.

* To undertake treatment planning and implementation of agreed goal-oriented interventions with client/carers.
* To contribute towards single assessment and trusted assessment processes.
* To provide comprehensive intervention to patients using available assessment and treatment facilities.
* To manage a clinical caseload within multidisciplinary teams, ensuring patients and carers receive optimum level of therapy and holistic care within available resources.
* To carry out multidisciplinary screening assessments and discuss outcomes with team members.

Detailed job description and main responsibilities

To lead and manage a community multidisciplinary rapid response and intermediate care team, taking overall responsibility for assessment, management and delivery of community rehabilitation, rapid response and intermediate care to patients in community settings, and to provide therapy input to Home First Service patients. To promote effective teamwork within the wider Home First Service team, delegating activities appropriately for patient benefit and service delivery.

Values and Behaviours

* AHP with appropriate Diploma/degree; current HCPC registration; further education in a relevant field; evidence of CPD relevant to older adults, physical disability and rehabilitation; up-to-date CPD portfolio.
* Qualification in team/service management or willingness to work towards it; Masters level study related to rehabilitation or working towards this.


Experience

* Recent post-graduate experience in core areas including elderly care, respiratory and rapid response; experience in community/primary care settings; experience in multi-disciplinary environments; experience in managing complex care plans; experience in supervising and teaching others; active participation in in-service training; experience in service development; experience in multi-agency working; experience promoting services within a locality; experience with intermediate care; experience with virtual ward models in community settings.


Skills and Knowledge

* Knowledge of care for long-term conditions and relevant standards (NICE, NSFs, etc.); advanced skills in assessment and treatment of adults with complex multi-pathologies; knowledge of current health care policy and primary care collaboration; knowledge of audit, evidence-based practice and clinical governance; ability to work autonomously and as part of a team; teaching and mentoring skills; ability to manage conflict; effective communication skills; basic IT skills including Word, PowerPoint, Excel and clinical recording systems.
