The role:
As Clinical Case Manager, you will join our network to support vulnerable children, young people, and adults following injuries like brain injury, spinal cord injuries, complex orthopaedic injuries, amputations, psychological injuries, and more. Your responsibilities include:
* Managing a caseload through rehabilitation.
* Leading assessments and goal setting at clients' homes.
* Signposting, advocating, and assessing needs for fulfilling lives, including accessing services, community reintroduction, work, or meaningful activities.
* Handling all case-related administration, instructions to third parties, and report writing/reviewing.

Skills and experience required:
* At least 3 years of clinical experience
* Valid registration with HCPC, NMC, or SWE
* Experience with rehabilitation packages
* Community or case management experience is advantageous
* Strong relationship-building skills
* Driver with own vehicle
* Willingness to travel extensively across the UK, Scotland, and Wales
