We are looking for a Physical Health Support Worker Lead to join our team at Cygnet Hospital Ealing who is experienced in working within general hospital outpatient clinics. You will be expected to work 40 hours a week (Monday – Friday, 9am – 5pm).
Responsibilities
* Embed physical health care policy and promotion and support GP, Clozapine and other physical health interventions.
* Support and assist patients with individual physical health care plans and provide input into these plans.
* Take bloods and provide support to service users anxious about their physical health.
* Monitor patients and perform basic health checks, coordinate a GP clinic and lead health awareness sessions with staff.
* Establish, lead and coordinate evidence‑based physical health practice initiatives across the service.
* Coordinate ongoing monitoring of physical health of all patients, including annual health checks in close collaboration with ward doctors and physical health nurses.
* Lead infection control practice for the hospital, including national clinical standards.
* Provide programme of health promotion and education in collaboration with dietician and ward physical health leads.
* Ensure patients have access to primary care services (dental, eye checks).
* Maintain knowledge of relevant legislation and guidance on physical health in mental health services.
* Contribute to national audits, carry out clinical audits, maintain comprehensive records in MyPath.
* Ensure access to required medical equipment and safe use of medical devices, educate staff and conduct audits.
* Ensure local protocols for the safe use of medical devices are understood and practiced by staff.
Qualifications and Experience
* Maths and English GCSE or equivalent.
* Further qualifications in health and social care or leadership (desirable).
* Current work in a healthcare and/or mental health setting.
* Evidence of continuing professional learning and willingness to train and develop new skills.
* Training in phlebotomy and/or other areas of physical health (required).
* Infection control lead training or willingness to undertake IPC course.
* Commitment to patient recovery and effective monitoring, management, and risk reduction.
* Successful candidates will be required to undergo an enhanced DBS check.
