You will enjoy a varied and stimulating caseload, working across multiple settings including a fast-paced GP Assessment Unit, Medical Assessment Unit, Bevan Court’s specialist frailty service with acute bedded care, and the Frailty Reablement Unit. Collaboration with our Admission Avoidance team will broaden your experience, including work in the Emergency Department, Frailty Ambulatory Clinic, Urgent Crisis Response, Community Frailty, and Virtual Ward services.
Your Impact
As an integral part of a multidisciplinary team, you will utilise your advanced clinical reasoning skills to holistically assess and treat frail patients and follow up. Your proactive interventions will help prevent crises, reduce hospital stays, and maximise patient independence, supporting people to live safely in their own homes wherever possible.
You’ll have the opportunity to:

* Collaborate closely with consultants, nursing staff, ACPs, and other therapists to deliver high-quality, coordinated care.

We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.

Detailed job description and main responsibilities
* To work within Trust clinical guidelines, HCPC and Registrant Body guidelines and to have a working knowledge of national and local standards and monitor own and others quality of practise against these
* To work in secondary care in a variety of hospital settings and the patient’s own homes.
* To demonstrate as appropriate to role highly developed physical skills for assessment and treatment of patients e.g. dexterity, co-ordination, palpatory and other senses.
* To deputise for senior staff in leading specific team.
