This is a unique opportunity to be part of a specialist team delivering holistic, high-quality care to young people with a range of palliative conditions and / or complex health needs ?? About the Role As a key member of our team, you will: Provide safe, effective, and person-centred care to young adults within our purpose-built Young Adult Unit and community settings. Coordinate and deliver individualised care plans, working in partnership with young adults, families, and external professionals. Mentor, train and assess the competence of Care Support Workers Act as a keyworker for a small group of young adults, ensuring continuity of care and advocacy. Participate in service development, audits, and educational activities to help grow and improve our service. Liaise with professionals across health and social care to ensure integrated, high-quality support. A role where no 2 days are the same; you may be supporting on a community outing, taking care of a young adult during a seizure, or developing your leadership skills by coordinating the shift. ??
