Main duties of the job
Care coordinators play an important role within a PCN by proactively identifying and working with practices to evaluate screening uptake and reduce health inequalities. They support the implementation of NHS England's Early Cancer Diagnosis Designated Enhanced Service by reviewing practice data and working with partners to improve screening program uptake. The post-holder will ensure patients receive appropriate information and support to book screening appointments, and assist practices in accessing resources tailored to their patient populations.

Purpose of the role
By 2028, the goal is for 75% of cancer patients to survive five years post-diagnosis, up from just over 50% now. Early diagnosis (stage 1 or 2) is critical, with a target of 75% of cancers diagnosed early by 2028.
Primary duties and areas of responsibility
* Assist the PCN in improving patient services related to participation in national cancer screening programs.
* Build supportive relationships with practice staff involved in cancer screening efforts.
* Help practices evaluate screening uptake and engage underserved populations to reduce inequalities.
* Support the organization of peer-to-peer learning events analyzing diagnosis and screening data.
* Source resources and training from organizations like Cancer Research UK, Macmillan, and local authorities.
* Develop and support implementation of cancer screening improvement plans for practices.
* Create a resource library with data packs and patient information in various formats, including adaptations for diverse patient needs.
* Review and standardize practice coding and reporting templates, liaising with Ardens for improvements.
* Provide administrative support for project evaluation and feedback.

* Previous admin and clerical experience within the NHS or similar field.
