An exciting opportunity has arisen for a Band 6 Community Night Nurse, tojoin our Farnham Community Nursing Teamto help us continue to change things for the better
The Farnham Community Nursing team is based at Farnham Hospital and covers Farnham and surrounding Surrey Heath areas. They are a highly supportive team who are happy to develop an aspiring Band 5 nurse into a Band 6.
The Night Community Nursing team covers 8pm to 8am, 365 days a year. We have full and part-time hours available.
Main duties of the job
Within this role you will:
* Provide leadership and management to your team.
* Have responsibility for your caseload and have skills and competencies to provide excellent patient care in their homes.
* Undertake full nursing assessments of patients, including those with multiple co-morbidities and complex health and social needs.
* Be responsible for agreeing the patients care plan, liaising with others and adapting the care and treatment delivered according to need and patient choice.
* Work with the Integrated care system and MDT to provide holistic care for your patients.
* Responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
* implement, monitor and maintain high standards of care delivery at all times.
* Care for patientsand their families in the way thatyou believe is best

Job responsibilities
You will be a NMC/1stlevel registered Nurse preferably some community experience.You will have the ability to work both independentlyand alsoas part of a wider multidisciplinary team. It is expected that youshould have strongcommunication and organisation skills and a flexible approach which will ensureyou deliver positive experiences to patients at every stage of theirhealthcare journey.You will have a full UK driving license with access to a car for work purposes.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packagesand haveworking knowledge of an i pad which is used for mobile working.
