Main duties of the job
To support the practice clinicalteam by signposting patients to the appropriate healthcare professional orservice, working as part of the practices multidisciplinary team. The carenavigator will also be expected to undertake reception duties as part of therole, supporting the administrative team in delivering a polite andprofessional service to the entitled patient population.

Job responsibilities
The following are the coreresponsibilities of the Care Navigator. There may be, on occasion, arequirement to carry out other tasks; this will be dependent on factors such asworkload and staffing levels:
Process and effectively signpostpatients to the appropriate healthcare professional depending on the presentingcondition
Answer incoming phone calls,transferring calls or dealing with the callers requests appropriately
Process patient requests forappointments
Initiate contact with and respondto requests from patients, team members and external agencies
Enter read-code data on SystmOne
Photocopy documentation asrequired
Data entry of new and temporaryregistrations and relevant patient information as required
Input data into the patientshealthcare records as necessary
Manage all queries as necessaryin an efficient manner
Carry out system searches asrequested
Maintain a clean, tidy, effectiveworking area at all times
Monitor and maintain thereception area and noticeboards
Support all clinical staff withgeneral tasks as requested

Experience
* Experience working in a customer/patient facing role.
* Experience working in a healthcare environment.
