Qualified Social Worker to join our team at AV Care Solutions Limited. This role involves working with vulnerable adults/children and families to assess needs, develop care plans, and ensure high-quality safeguarding support in line with statutory requirements.
The position offers a hybrid working arrangement, allowing a balance between office-based duties, community visits, and remote administrative work.
Key Responsibilities
* Conduct assessments and reviews of service users’ needs
* Develop, implement, and review care and support plans
* Work in line with safeguarding policies and procedures
* Liaise with families, healthcare professionals, and partner agencies
* Maintain accurate and timely case records
* Participate in multidisciplinary meetings and case conferences
* Ensure compliance with social work legislation and council/agency standards
Requirements
* Qualified Social Worker (Degree in Social Work or equivalent)
* Registration with Social Work England (SWE)
* Experience in children’s or adult social care (depending on team)
* Strong assessment, communication, and case management skills
* Ability to work independently and as part of a team
