Overview
An opportunity has arisen for a Support Planner to work within Caring For Thurrock. The role is to work alongside team members who are responsible for managing the staff and service users within the service. Caring for Thurrock is the internal care department of Thurrock Council Adult Social Care and is responsible for provision of Care for Thurrock residents. The team is a Multi Disciplinary Team working alongside colleagues within the Health Service in providing long term and reablement care to service users within Thurrock. As part of the Caring For Thurrock team you will be working in a way that is responsive and proportionate, working with people to achieve their outcomes using their strengths and their community creativity, linking people with their communities focusing on what matters rather than what\'s the matter.
You will need to be an excellent communicator and able to work alongside colleagues in other departments, health, community partners and agencies in order to meet the care and support needs, and outcomes for our residents. Responsibilities
* Work alongside team members responsible for managing staff and service users within the service.
* Collaborate with colleagues in health, community partners and agencies to meet care and support needs and outcomes for residents.
* Contribute to a responsive and proportionate approach to social care aligned with the team\'s approach to safeguarding and service delivery.
* Engage in safeguarding processes as part of the Safer Recruitment framework (Gaps in Employment form may be required).
Qualifications and Requirements
* Evidence of essential qualifications listed in the Job Profile and Person Specification (if applicable) and upload copies of those relevant qualifications.
* Disclosure and Barring Service (DBS) check at enhanced plus barred level is required for this role (at no extra cost).
* Ability to communicate effectively and work with multiple stakeholders including other departments, health services and community partners.
