Main duties of the job
Working life

Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities.
The service aims to provide an excellent standard of care that supports patients to remain in their home environment. In Northamptonshire you will work as part of a multi‑disciplinary team to support the service and focus on delivering quality outcomes and improving the patient experience. You’ll play a vital role in keeping hospital admissions and readmissions to a minimum and ensuring that patients can return to their own homes as soon as possible.
Must have skills
You will need to be adaptable and resourceful. You’ll need to be well organised, confident and able to cope in diverse situations to deliver patient care. Leadership and management skills are also important, as with all nursing roles, communication skills, problem solving and good judgement are crucial. You will be part of a caring team and be able to work flexibly with minimum supervision, with the ability to prioritise work based on the patient need.
Person Specification
NMC Registration
* NMC registered adult nurse with Post Registration Community Nurse experience
* Working Knowledge of System One Patient Electronic Record
* Good Communication and interpersonal skills
* Able to work autonomously and independently
Community Nursing Experience
* Experience in Community
* Experience in Venepuncture
* Palliative Care
* UK car driver and owner
