The Intensive Home Nursing Service is a service provided by Sheffield Teaching Hospitals Foundation Trust IHNS provides short term care for patients who are approaching end of life and wish to spend their remaining time in their home.
The direct care is provided on a one-to-one basis by Band 3 Clinical Support Workers with support from other health and social care teams. The Clinical Support Workers are directly managed and supported by the nursing team within the Intensive Home Nursing Service.
In this role, you will be required to:
To carry out patient assessment visits in the community setting.
To assess, plan, implement and evaluate care required by the patient, ensuring that the service criteria is being met and that the documentation on SystmOne is up to date and correct.
Undertake thorough risk assessments regarding suitability and safety of patients dwellings prior to allocating care from the band 3 CSWs.
To be trained as a transcriber to ensure all relevant drug administration charts are completed accurately and safely.
To ensure that all medication charts are correct and up to date.
Provide supervision and support to the Band 3 clinical support workers to maintain high quality effective health care.
Main duties of the job
Assess, Plan, Implement and Evaluate Patient Care in a community based setting.
Support the duty Coordinator in organizing and deploying staff according to competencies and workloads in order to meet patients’ needs.
To be part of the continual assessment of care needs and liaise with other multi agency professionals as appropriate. Report any concerns to senior staff nurse or team leader.
To support the senior staff nurses in the effective management and use of resources to ensure a reactive service.
To provide training and support to the Band 3 Clinical Support Workers in the home environment and at the monthly support and education sessions.
To ensure that planned nursing actions are delivered safely, effectively and are evidence based.
Assume responsibility for own professional development.
To ensure that clear verbal, written and digital communication takes place with a range of people, including Community Nursing Teams, St Lukes Hospice, Continuing Health Care colleagues, Transfer of Care team, Active Recovery, GPs, inpatient areas and voluntary sectors as well as patients, carers and relatives.
To participate in multi-disciplinary team meetings, daily Palliative Care Huddles, CHC huddles and case conferences.

This vacancy is advertised on a fixed term contract basis for a period of a maximum of twelve months due to maternity leave of the substantive post holder. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and statutory maternity provisions.

Essential criteria
1. Professional Nursing Qualification, degree or equivalent experience.
2. Current NMC registration.
Desirable criteria
3. Leadership/management knowledge through formal study/short courses
4. Palliative care knowledge through formal study/short courses
5. Teaching and Assessing Course or Equivalent or working towards.
Experience
Essential criteria
6. Significant post registration Band 5 experience
7. Ability to assess, plan, implement and evaluate care.
8. Experience of palliative / terminal care.
9. Experience of working in a multi-disciplinary primary health care team.
10. An understanding of health, social and voluntary services.
Desirable criteria
11. Post registration primary care experience.
12. Experience in co-ordinating and prioritising own workload and ability to delegate appropriately.
